Healthcare providers, schools, licensed childcare centers, health plans, and other organizations listed in Minnesota Statutes, 144.3351 Immunization Data are encouraged to participate in the Minnesota Immunization Information Connection (MIIC) to support immunization practice, monitoring, and improvement in Minnesota.
Organizations not eligible to participate in MIIC must receive consent from a person for MDH to release their immunization information to the organization.
VaxCheck is a tool to collect consent to share immunization information from MIIC with organizations.
Organizations can use VaxCheck to invite employees to consent to MDH sharing their immunization data with their employer. Because directly accessing an employee's immunization history is not a permitted use of MIIC, VaxCheck provides a compliant alternative for organizations that require proof of vaccination.
Certified childcare centers can use VaxCheck to invite parents or guardians to consent to MDH sharing their child’s childhood immunization information with the center where the child is enrolled.

VaxCheck process
Organization requests to enroll in VaxCheck at VaxCheck Portal.
MDH reviews and approves the enrollment request.
Organization sends consent forms.
Each organization is assigned a unique link for the consent form.
The link to the consent form can be emailed directly from within VaxCheck or through your organization's existing email processes.
For each consent form signed, the individual is matched to a MIIC record.
Organizations download a spreadsheet from VaxCheck which will show:
Who has signed a consent form.
If that individual matches to an existing MIIC record.
The applicable immunization information, if there is a matching MIIC record.
