Each home care provider must develop and implement an individual abuse prevention plan for each vulnerable minor or adult for whom home care services are provided by a home care provider. The plan shall contain:

the person's risk of abusing others (including self-abuse)

No. Although in some situations it may make sense for the nurse to do so along with the nursing assessment, this is not required. Statute refers to it as either an individualized review or assessment.
