Biannual reporting

Deadlines: December 2025

Reporting will open in REDCap on December 1, 2025, and is due by December 31, 2025. The following reports are due within the REDCap project titled "CDC Federal Infrastructure Grant":  

1. Reporting Job Classifications Due December 31, 2025
2. Reporting Job Program Areas Due December 31, 2025
3. Annual Progress Report 2025

Update to December 2025 reporting: Only count positions supported by CDC PHIG funding at the end of the reporting period

Updated reporting guidance from CDC indicates a change from previous reports: Only the total number of positions supported by grant funds at the end of the reporting period should be counted. This means if a position is no longer being supported by grant dollars on November 30, 2025, the position should not be counted.

MDH will offer short webinar/office hours at the following times to discuss updates to reporting. Please plan on attending one of these sessions. Time will be available for questions.

• Wednesday, December 3 (10:00-10:30 a.m.)
• Thursday, December 4 (2:00-2:30 p.m.)
• Monday, December 8 (10:30-11:00 a.m.)
• Wednesday, December 10 (3:00-3:30 p.m.)

  Watch this webinar for a refresher on how to complete the required reports, completed every six months, including the two hiring reports (Job Descriptions and Program Areas).

  Reporting will involve a “hiring report” due June 30 and December 31 each year of the grant (2023-2027) and an Annual Progress Report due December 31 each year of the grant (2023-2027). During the webinar MDH walked through what these reports entail and how to complete them in REDCap.

Minnesota was awarded $42.9 million for the Strengthening the U.S. Public Health Infrastructure, Workforce, and Data Systems Grant (CDC Federal Infrastructure Grant). As outlined in grant guidance, community health boards will receive 40% of the funds awarded to Minnesota for the workforce area of the grant. This is $16.4 million to community health boards for use over a five-year period.
