MnVFC Required Reports and Trainings

Each site that stores and/or administers Minnesota Vaccines for Children (MnVFC) vaccine must enroll in the MnVFC program and have their own Personal Identification Number (PIN). Sites are responsible for maintaining their enrollment in the program by completing the requirements below.

MnVFC policies and procedures manual

The 2026 MnVFC Policies and Procedures Manual (PDF) for both Replacement Method and Separate Stock sites has been combined into one manual this year. Please ensure that you are paying close attention to information for your site type. Especially in the How to Manage the MnVFC Program section. The complete guide to MnVFC program policies and procedures. Sites should review the 2026 manual and sign and date the signature page (page 7), certifying they have read the manual. The blank pages in the manual were purposely left to allow for two-sided printing.

If your browser loads a previous version of a document, clear your cache or use a different browser. If you are using Internet Explorer, Edge, Google Chrome, or Mozilla Firefox you can quickly clear cache with a keyboard shortcut. While in your browser, press Ctrl+Shift+Delete simultaneously on the keyboard to open the appropriate window. You can also get to this through the settings menu in most browsers.

MnVFC annual online training

This training is required for the vaccine coordinator and backup vaccine coordinator by November 30 each year.

To access the course, go to Minnesota Department of Health (MDH) Learning Center and follow the instructions on the page.

Once you have logged into the Learning Center select Catalog, Infectious Disease, and MnVFC Program Required Annual Training 2026, or you can just search for MnVFC. When you find the course, select click to enroll, start (to load the training), and the play button to begin.

Remember to print the certificate of completion, keep it with MnVFC paperwork, and have available for site visits.


Required forms and reports

Links to complete the forms required for your site will be emailed to your MnVFC site contacts each fall.

  • Minnesota Vaccines for Children (MnVFC) Program Provider Agreement: The provider agreement must be signed by the medical director every 24 months (by November 30 in odd years).
  • Minnesota Vaccines for Children (MnVFC) Program Annual Report of the Number of Immunized Pediatric Patients: The annual report is due by November 30 each year. Sites must report a de-duplicated count of pediatric patients immunized from October 1 of the previous year through September 30 of the current year.
  • Replacement sites ONLY: Replacement Method of Vaccine Management Agreement: Sites that use the replacement method of vaccine management are required to complete an additional form each year. It is due by November 30. Parent sites may sign a replacement agreement on behalf of all sites in their system, but the agreement must include a list of each PIN number for it to be valid.
