Nursing Home Discharge Appeals

Residents of nursing homes who are being involuntarily transferred or discharged by the facility have the right to appeal the proposed transfer or discharge.  To appeal, please complete and submit an HRD Appeals Submission Form:

• Choose “Vulnerable Adult” from the Appellant Identity dropdown
• Provide the vulnerable adult’s name and contact information. If another person is a better option for contact, provide their contact information instead.
• If an ombudsman, case manager, or attorney is assisting in the appeal, answer “Yes” to the question “Are you represented by an attorney?” Provide their name and contact information in the fields that appear.
• Choose “Nursing Home Discharge Appeal” from the Type of Appeal dropdown
• Provide the name of the assisted living facility.
• Provide a brief summary of the reason for your appeal.
• Please attach a copy of the discharge notice issued by the facility, if available. 

The appeal must be submitted within 30 days of receiving the discharge/transfer notice.
