How to report fraud

Submissions through this form are reviewed for matters involving MDH employees, programs, operations, vendors, grantees, contractors, and potential fraud, waste, or abuse tied to MDH activities or funding. 

You are not required to provide your name or other contact information, but without that information we will be unable to consult with you about your allegation.

Submissions using this form that are unrelated to MDH business operations may not be reviewed timely or routed for follow-up. For general public health or licensing complaints, please visit the appropriate complaint program listed below.

What is fraud

Fraud is an intentional or deliberate act to take money or property from another person through deception or other unfair means. It includes knowingly using false information or committing a crime in order to obtain money, a benefit, not public data, or services from the state to which the person is not legally entitled.
