The State of Colorectal Cancer in Minnesota
March 19, 2024
This webinar from the MDH Comprehensive Cancer Control Program (CCCP) highlights data on colorectal cancer incidence and mortality, and screening rates statewide and among populations experiencing the largest disparities for colorectal cancer. The CCCP team shares current guidelines and options for screening, and tools to support or improve colorectal cancer screening in clinics. Two primary care clinics also share ways that they have improved their screening rates.
Learning Objectives:
- Participants will be able to describe the populations more heavily burdened by colorectal cancer in Minnesota.
- Participants will be able to clearly outline to their patients the current screening guidelines and options for colorectal screening.
- Participants will be able to come up with 1-2 ways that they will enhance or implement changes in their clinic to improve colorectal cancer screening rates.

Care Coordinators: Bridging Care for Diabetes Management
February 8, 2024
Diabetes self-management is complicated, and supporting patients with diabetes can be challenging. Bridget Ideker from MDH explored these challenges and provided practice-based learning, tools, and resources to address them.
Learning Objectives:
- Participants will be able to state 2 ways to use the PAID scale for patients with diabetes.
- Participants will be able to define diabetes distress.
- Participants will be able to state how to refer diabetic patients to Diabetes Self-Management and Education Support (DSMES).

Developing Innovative Models of Care Post-COVID

Zora Radosevich from the Minnesota Department of Health Office of Rural Health and Primary Care kicks things off with a short presentation, then leads a panel discussion with Health Care Home providers from Stellis Health, Riverwood Healthcare, and HealthPartners.

Using the Connection Model to Bridge Clinic and Community

This webinar is a continuation of the webinar Overcoming Socioeconomic Barriers in Rural Minnesota Using a Connection Model. In this session, Dani Protivinsky, CentraCare, explores where and how to start building your own unique partnerships at the clinic and community level. Learn how to address social determinants of health and play a pivotal role in patient outcomes. Discover how the connection model and other resources, including the Take 5 model, can help you identify gaps and opportunities in practice and partnership to roll out your own plan of intervention.

Overcoming Socioeconomic Barriers in Rural Minnesota

Dani Protivinsky, Director of Health Equity and Community Health Improvement at CentraCare, shares how their team of nurses, health educators, and staff members used culturally and linguistically appropriate connections and creative interventions to gain patient and community trust while reducing disparities.
